| Abstract: | We describe a case of Hansen's hernia myelopathy type II in a mongrel female canine of 8 years old, who was admitted for emergency consultation, due to posterior paralysis, the owner reports that she was in the park when her pet collapsed making a vocalization in pain and could not support herself with her hind limbs, she also refers to a fall from the stairs a year ago. Clinical examination revealed a loss of proprioception of the hind limbs, loss of superficial and deep sensitivity, decreased pannicular reflex with presence of pain in the thoracolumbar region. The patient was admitted for inhospital management; as a diagnostic plan, thoracolumbar and lumbo-sacral spine X-rays were taken with L-L views showing lumbosacral stenosis and decreased intervertebral space between T11-T12 and L2-L3. Paraclinical tests were taken, including a hemoleukogram, liver and renal function markers, which were normal; The patient was referred to the diagnostic imaging service to perform a contrasted nuclear magnetic resonance of the thoracolumbar and lumbosacral spine, resulting in a decrease in volume and an irregular dorsal border in the disc corresponding to the T12-T13 space, so a Hansen hernia type II is suspected; associated with a moderate increase of signal in T2 and STIR sequences in dorsal medullary parenchyma, compatible with an inflammatory and/or degenerative process. Likewise, the disc corresponding to the segment between L3-L4, show ed a decrease of heterogeneous signal and a slight dorsal protrusion, generating meningeal indentation. It was decided to perform in-hospital management for seven days where the patient showed recovery of sensitivity and reflexes to a lesser degree, a therapeutic plan was initiated with medication at home and physiotherapy in range of motion and laser therapy. The patient evolved recovering the mobility of his hind limbs, as well as the neuropathic pain he presented. |
